*QUALIFICATIONS:*
* Previous experience in a Long Term Care Facility.
* Knowledge of Wound Care practices.
* New York State Licensed Professional Nurse, minimum of Bachelor’s Degree, MDS Knowledge, Wound and Infection Control knowledge, Care Planning and Assessment knowledge
*GENERAL OBJECTIVES: *
To ensure residents receive wound care services in order to meet their highest practicable level of physical functioning.
*RESPONSIBILITIES RELATED TO WOUND CARE:*
1.
Supervises, monitors, and directs the activities of all nursing personnel to ensure the
provision of wound care to the residents of the facility.
2.
Assesses residents to determine their risk factors for contractures, skin breakdown and
deterioration in ADL’s in order to develop a nursing rehab plan of care.
3.
Monitors the effectiveness of the plan of care through direct observations of care rendered and through weekly wound rounds/ reassessment.
4.
Develops a plan of care for each resident with a current pressure sore, or evaluated as high risk for the development of pressure sores.
5.
Provides leadership by working cooperatively with ancillary nursing and the interdisciplinary team in maintaining standards for professional nursing practice in the clinical setting.
6.
Coordinates and performs physician-ordered patient wound care following the wound care guidelines.
7.
Active role in identifying and addressing quality issues pertaining to development, prevention of wounds, and implementing corrective action and plans as necessary.
8.
Supports and maintains Nursing Services objectives and standards pertaining to Wound
Care
9.
Completes Wound care, positioning audits, analysis of trends, etc.
as assigned by the
Quality Assessment and Assurance Coordinator.
10.
Maintains current list of residents with wounds and their treatments.
11.
Monitors EMR dashboard’s “Alerts” pertaining to Wound Assessments.
*RESPONSIBILITIES RELATED TO RESTORATIVE NURSING*
Refers any deterioration/improvement in ADL’s/cognition to the MDS Coordinator for assessment of a significant change.
* Assesses all residents upon admission/readmission in order to ascertain their status related to ambulation, range of motion, bed mobility and transferring, eating, toileting skills and use of adaptive devices.
* Assesses residents who have been discharged from restorative/maintenance rehab therapy in order to continue with their achieved skills on the nursing units.
* Works in close coordination with Rehab Services in order to ensure a continuum of therapy’s goals are carried out on the nursing units.
* In-services nursing caregivers each resident’s specific nursing rehab needs.
* Ensures nursing meets the positioning needs of resident’s not able to position themselves in bed and in chairs.
Performs any and all duties related to the effective and efficient operation of the institution as may be assigned by the Nursing Services Administration Staff.

Pay: $105,000.
